Eligibility for this Scholarship Program is restricted to the following:

Current of Former WPSD Employee including Fire, Police, EMS, Police Reserves, Police Explorers, Fire Explorers, and Administrative Services.

Immediate Family Member of a current or former WPSD employee and pursuing certification, or post-secondary education in a Medical or Emergency Services field.

You must disclose all forms of support you will receive for the eligible course of study (e.g., scholarships, grants, gifts, tuition reimbursement, etc.).

- I hereby authorize the release of this application and relevant supporting information to members of the WFFAA Scholarship Committee for their review.

- I understand that all the information contained in this application is considered confidential and will only be viewed by the WFFAA Scholarship Committee for the sole purpose of determining scholarship eligibility and awards. I certify that the information that I have provided in this application is true and accurate, to the best of my knowledge.

- I understand and agree that the purpose of the Scholarship Program is to defray the cost of tuition and any Scholarship award will be paid directly to the Academic Institution that I am enrolled in.

- I understand and agree that I am obligated to notify the WFFAA and repay Scholarship award monies if I change majors or discontinue my course of study within the year that the Scholarship was awarded.

- I hereby give permission to use photos of me and general, non-financial information included with this application, for WFFAA publicity and announcement purposes.
